Financial AssistanceThe Dubuque Community Y charges fees for services, but it is our policy that financial assistance is available to those who qualify based on family size and income guidelines. You may call or stop by the Y Main Desk to pick up financial assistance application or submit the following information by mail: • Include the names and birth dates of all family members residing in the household. • Include your gross family income: wages, salary, tips, etc. • Include any benefits you receive such as unemployment, social security, retirement, etc. • Include other assistance such as child support, food stamps, etc. • Include any other information such as medical hardships that your family is incurring. • Print your name, mailing address, and phone number along with your application. • Attach a photocopy of a recent check stub, W-2 form, SSI statement, or other verification. • Please submit your typed or printed application for financial assistance by mail to: Membership Director • Please allow up to two weeks for notification of your eligibility for assistance • For More information call the Dubuque Community Y at (563) 556-3371
